We are asking all parents and guardians of students at Claremont Middle School to fill out this year’s California Healthy Kids Survey (CHKS), a statewide survey focusing on the relationship between students’ health behaviors and academic performance. This simple five-minute survey is an opportunity for you to help our school by sharing your opinions about school climate, discipline, academics and parent involvement.
No one can tell who filled out this survey, so your answers will be completely private and anonymous. If you have more than one child at Claremont, you may fill out the survey separately for each student, or just focus on the experience of one of your children at Claremont. More than one caregiver may fill out survey as well.
